Provider Agencies are businesses that provide services to DD/ID individuals.

The biggest service that they provide is individual, in-home and community care. This means that they will send a care provider to an individual's home to help them work on meeting the goals they had previously set up with their Service Coordinator or Personal Agent.

Goals may include:

  • Completing household chores

  • Grocery shopping

  • Using the bus system

  • Managing a budget

  • Communication and social skills

  • and much more....

Some Provider Agencies will also have other program perks such as group outings and on-site activities with other people in their programs. Case Management Services (Brokerages)

Brokerages provide persons with intellectual and developmental disabilities with Service Coordinators or Personal Agents. You will need a SC or PA in order to access any programs or services in Oregon. Some services they help connect you with are:

  • Personal Support Workers

  • Provider Agencies

  • Job Assistance

  • Day programs

  • Housing support

  • Personal equipment and supplies
